Insurance and Payment Options
Most insurance plans are accepted including - deductibles and co-pays may apply
- United Health Care including UMR
- Gundersen Health Plan and Quartz
- Anthem and Blue Cross/Blue Shield
- Medical Assistance - Must be in the Gundersen/Quartz/BCBS HMO - if you are unsure leave me your Forward ID number and I can check it for you
- Optum
- WEA
- WPS
In order to find out if we are a provider for you just call the number on the back of your insurance card and ask if Ashley Grimsled or Counseling Assoicates, LLC are in your network of providers. Let me know if you have any questions or if I can be helpful with this process.
At this time we are unable to accept Mayo and/or Health Traditions Insurance plans - even the ones through Badgercare.
We are also able to offer a self-pay options of $100/per session with payments due at time of service.
Please be sure to bring your insurance card with to your first appointment. Co-pays may be collected during your visits.
The initial paperwork can take 10-15 minutes to finish so please arrive early to your first appointment. The parking ramp on Jay Street between 4th Street and 5th Ave. offers free parking for the first three hours if you cannot find street parking.
